If your PPO or worker’s comp plan covers acupuncture for out of network providers AND will reimburse the provider directly, you may submit an insurance verification form (in New Patient Forms) for consideration.
As an alternative, you can email me a photocopy of your insurance card (front and back), along with your name and date of birth.
